Does every employee need an employment contract in Nepal?

Generally, yes. Section 11 of the Labour Act prohibits an employer from employing a person without an employment agreement, except that a written agreement is not required for casual employment.

The agreement should state remuneration, benefits, employment conditions and other prescribed matters. An appointment letter also falls within the Act's definition of an employment agreement.
